Please use the guidelines below instructed by District Office.
ALL PARTICIPANTS WILL NEED TO CREATE A “RANK ONE” LOG-IN TO COMPLETE THE DIGITAL DOCUMENTS AND UPLOAD PAGE 5 TO THE NEW DIGITAL PLATFORM!
Returning Athletes
In addition to completing the necessary steps for rank one, once your doctor completes the DIAA physical, you should turn in only Pages 2 & 5, “Consent Form” and “School Medical Card” to Mount Pleasant.
9th Graders, New Students, and First Time Athletes*
In addition to completing the necessary steps for rank one, you will need to submit the whole DIAA physical packet and vaccination information to Mount Pleasant.

Make sure to complete all areas of the physical and sections in Rank One or it will be marked incomplete.

We are excited about what our Student-Athletes and Coaches are doing in preparation for a very successful season in the Flight-B Conference. We are excited to be competing against some new programs and appreciate the continued competition with our familiar ones.
PRESEASON IS COMING!
IMPORTANT REMINDERS:
1. All Athletes must be enrolled as a 9th - 12th grade student at Mount Pleasant to qualify to tryout.
2. All Athletes must have a current DIAA physical dated after April 1st, 2026 to qualify to tryout.
3. All Athletes must be academically eligible to qualify to tryout.
**Athletes not meeting any of these three requirements are not permitted to participate in any team activities.

BUS INFORMATION – For those that require transportation, there will be a city/suburb bus run every morning and afternoon August 17th - August 18th. The first stop will be at 6:30am and the bus will arrive by Mount at 7:15am and return to take the athletes home at 3:00pm. Please check greenknightssports.com for updated times and changes. CITY PICK-UP/RETURN – In the morning, please be at the stop ten minutes before the listed pick up time in order to avoid any issues.

2026 - 27 DIAA SPORTS PHYSICAL INFORMATION
I. Fall sports begin Monday, August 17th. No student can participate if they do not have a cleared DIAA physical on file with the school nurse.

II. Physicals can be submitted in the following ways:
- Bring the COMPLETED physical form to the main office and place them in the box labeled DIAA Physicals. These will not be checked until the week of August 4th. Parents will be notified then of any needed corrections to forms.
III. Wellness Center will have dates after July 29th, by appointment or walk-in. Must be registered as a current Mount Pleasant student.

4.. If you have already submitted the form you can check if there were any mistakes or if you are on the completed list.
5.. You can correct the mistakes and turn the form back in or you can take it with you to get the proper information or signatures.
